I’ll be visiting Tuscany with my girlfriend in September and am hoping to do (at least) one day of tasting at a few wineries in Montalcino. We’re hoping to find a guide of sorts who can take us between a few producers/generally show us around and help us schedule a few visits. Most interested in visiting Il Poggione, Poggio di sotto, Biondi-Santi (of course, but know this is unlikely), Altesino, Fuligni, and Casanova di Neri.
Has anyone used or is anyone aware of a service/individual like this in Tuscany that they can recommend? We’re not dead set on the producers listed, just a few examples! 
I visited Poggio di Sotto last year and drove. Beautiful winery, a bit pricey but the experience was worth it and they poured the Riserva which was not on the list to be tasted.
See if Grape tours out of Florence can accommodate. I’ve used them in a group setting previously.

Direct email request. Yes, very easy to get an appointment although we went in June prior to harvest time. They will likely be in harvest mode, but still worth the ask. Ciacci Piccolomini is right down the hill from them as well and actually used to own the land where Poggio di Sotto is now at.
I visited Costanti a few years back as well, great wine from the estate.
I recently booked a variety of visits in Montalcino and Montepulciano. Pretty much all require an initial inquiry email. Some respond very quickly, others may take a few days. After that the larger producers tend to have a web portal that they send you to with tasting options, prices and a booking calendar. Smaller operations are simply a couple of emails exchanged to reserve a date and time. In the latter cases you might not even know what will be served and whether or not there will be a fee.
Wife and I drove from Florence. Had great time visiting Conti Costanti (Monralcino) and Castello di Monsanto (Barberino Val d’Elsa).
I simply emailed them beforehand, as directed by their websites.
